ToolCallCompletedEvent

A tool call has one pre-terminal lifecycle notification, for execution starting or approval being required, followed by one terminal outcome. This pairing includes calls that require human approval. The terminal outcome is COMPLETED, FAILED, CANCELLED or TIMED_OUT.
